PROJECT SUMMARY / ABSTRACT Almost 2 million Americans are hospitalized with sepsis each year, and nearly one in five don’t survive. Most efforts to reduce sepsis deaths begin with the premise that patients are largely similar, and that targeting treatment to a single mechanism will improve outcomes. But sepsis patients differ in pathogen, biology, outcomes, and are proposed to have treatment responsive subtypes. As more and more sepsis subtypes are proposed, there is a stalled effort to translate these to the bedside for patients. To solve this challenge, we will use a clinical remnant biorepository to validate mechanistic sepsis subtypes, and combine existing approaches into subtype ensembles. Rather than chose a single best classification scheme, we use clinical data, biomarkers, proteomics, metabolomics, lipidomics, and microbiome measurements to build layers of parsimonious subtypes. Next, we will extend models that link subtypes to treatment, using novel reinforcement learning methods. Finally, we will use extensive in silico modeling to understand how to apply subtype-treatment models at the bedside, ranging from traditional clinical trials to on- policy learning methods. Thi…
